<strong>Adverse Event<\/strong> is an event in which care resulted in an undesirable clinical outcome-an outcome not caused by underlying disease-that prolonged the patient stay, caused permanent patient harm, required life-saving intervention, or contributed to death.<\/p><p>Despite the implementation of all preventive measures and actions, the occurence of adverse events associated with patient care and safety cannot be completely precluded.\u00a0<\/p><p>It is therefore important to implement the culture of reporting adverse events among all stakeholders.<\/p><p><strong>Beau
